Dear Parents/Guardians,

 The school nurses of the Southwick-Tolland Regional School District welcome you to school.  We look forward to working with you to accommodate your child’s health needs while at school.  Health information is included in the student handbooks of each school.  Please refer to them for more detail. Feel free to contact the school nurse in your child’s building with any questions, concerns or to obtain necessary forms. Forms for medication administration at school and private MD physicals may also be downloaded from this site. Here is a review of the requirements for immunizations, physical exams, field trips and medications.

H1N1 Information

IMMUNIZATIONS

             The Mass. Dept. of Public Health requires that all students be fully immunized against measles, mumps, rubella, polio, diphtheria, tetanus and pertussis. The HIB vaccine is required for preschool. Physicians are well aware of these requirements as well as the required time intervals between doses. The State Department of Public Health mandates the school nurse to review the time intervals between the doses of specific vaccines. If the intervals between doses are outside of the Department of Public Health guidelines an additional dose of a vaccine may be required. Parents will be informed individually if this is necessary.  Every child must be properly vaccinated to attend school in Massachusetts. Exemptions may only be accepted for medical or religious reasons and must be placed in writing.

* A student must have the varicella vaccine or a physician certified history of chicken pox disease.  

 

 

PHYSICAL EXAMS

State regulations “require physical examinations of all school children within six months before entry into school or during the first year after entrance, and at intervals of either three or four years thereafter.” (The Comprehensive School Health Manual, 1995, Massachusetts Department of Public Health, p. 5-24

The Southwick-Tolland Regional School District requires physical examinations in grades K, 4, 7 and 10. Because your child’s physician has a comprehensive knowledge of the health status of your child, private physician examinations are strongly encouraged. If the school nurse has concerns about a student’s health she may require a student to have an exam to attend school. Specific exam requirements for each school.

WOODLAND  -    Kindergarten, 4th grade and new students must have a physical exam within

                               six months prior to beginning school. Notices will be sent home to   

                               parents/guardians of students who are not in compliance. Students who

                               do not have a physical on file will be examined by the school

                               physician.

 

POWDER MILL  - Students entering Grade 7 and new students are required to have a 

                                  physical exam. Notices will be sent home to parents/guardians of

                                  students who do not have a physical exam on file. A student who

                                  does not have required physical will be  called to the Health Office

                                  for a physical by the school physician.

                                  Pre-employment physicals may be held in June depending on the demand. 

                                  The student and employer need to complete yellow card” available at the

                                  Superintendent’s office.

 

HIGH SCHOOL  -  Grade 10 and new students are required to have a physical exam.

                                  School physicals are usually held in November. Notices will be sent

                                  home to those who are not in compliance and the student will be

                                  called to the health office for a physical exam if one has not been received.

 

 

HIGH SCHOOL ATHLETIC PHYSICALS

 If your child will be participating in sports he/she must have a physical exam performed no earlier than  June 1st of the coming school year.  (See High School Student Handbook). The school nurse must have a copy of this exam on hand to clear a student to participate in any athletic team. It is best to plan to have him/her examined by their private physician.

 

 

MEDICATIONS

The following policies are in place to ensure the health and safety of children who need medication during the school day. The following forms must be on file in your child’s health records before any medication can be administered at school.  The forms are available from the nurse and may be downloaded from this site.

 1.       A signed consent by parent or guardian to give medicine.

2.       A signed medication order completed by your child’s licensed prescriber (physician, nurse practitioner, etc.)  This order must be renewed at the beginning of every school year.

 The policies include over the counter medications such as acetaminophen, cough drops and ointments.  Medicines must be in a pharmacy or manufacturer-labeled container. Children at Woodland and Powder Mill are not permitted to transport any medicine to or from school. Medicines must be delivered and picked up by a responsible adult. No more than a thirty-day supply should be delivered to the school. High School students who self-administer may carry only a one-day dose of medication. High School students will be monitored by the School Nurse periodically to ensure they are carrying and using their medications safely and properly. It is necessary to obtain new orders and parental permission every year. Any medications not picked up by the last day of school will be destroyed.

 

FIELD TRIPS

Permission forms for field trips are sent home by the students’ teachers prior to the trip with a deadline for returning them, usually a week before the trip.  Please complete the form as soon as possible. You must write any medication your child may need in the large box on the field trip form and indicate if you give the teacher permission to administer the medicine on the field trip.  Be sure to write any medical condition in the box also (i.e. diabetes, migraine headaches, motion sickness, OR reactions to bee stings etc.). It can take some time to make necessary arrangements for your child’s safe enjoyment of a field trip.  Your cooperation is essential. Please discuss field trips with the school nurse at any time. (If your child has an order on file for a medication that it is not on the form, the nurse will do her best to contact you.) It is necessary to go through this process for every field trip.
